REGISTRATION

Only a legal parent or guardian may register a student for school. The first step in the enrollment process at SLA is to submit a Middle/High School Reassignment form requesting to attend outside of your districted school. This form is available through our front office and must be approved by our Principal and the Director of REA at the school district before a student can be admitted. Upon approval, the following documents and paperwork must be submitted:

FROM OUT OF STATE, COUNTY, HOME, OR PRIVATE SCHOOL

Official Birth Record -

Certified Birth Certificate
Passport or Certificate of Arrival
Proof of Immunization verified with Blue HRS Form 680
    (This form is available from a private physician or the Health Dept.)

Physical Exam

Within last 12 months indicating body parts checked, signed and dated by the physician. If there are any health concerns, an an authorization will be required for the administering of medication or accomodations.

Proof of Custody

If a parent is separated, divorced or another custodial situation applies, a copy of the most recent legal documentation is required.

Parent & Student Contracts

Provides an agreement to support the guidelines of our school.

Copy of Most Recent IEP, EP or 504 Plan

Most Recent Grades and Test Scores
